Waldeck Trust Home Care

The Department of Community Services is pleased to offer home care service for Bay Village seniors! Thanks to a generous bequest from W Wayne Waldeck in honor of his parents, funds are available to help subsidize the cost of home health care.

The Department has contracted with Griswold Special Care to provide services.

Application Process

Complete and submit a Waldeck Trust Home Care Service Application (PDF); note that applicants must reside in Bay Village.

A preliminary assessment of services and financial eligibility is completed by the Community Services Director, and once need and eligibility has been established a nurse from Griswold will complete a medical assessment and set up services.

Services

Since funds are limited, there is a maximum of 6 hours of service per week. Included services:

  • Bathing
  • Changing bed linens
  • Laundry
  • Light housekeeping
  • Meal preparation

Payment

Payment for service is determined on a sliding scale based on limited income guidelines:

  • A single person household with a monthly income up to $2,346 would be eligible for some help in paying for services.
  • For a two person household, the maximum allowable income per month is $3,157.

Additional Assistance

For those needing more assistance, a referral can be made to PASSPORT, a state program which pays for home health care with Medicaid funds.
